Case 75 Diagnosis

Acute exudative polymorphous vitelliform maculopathy OU

Differential Diagnosis

  • Harada’s disease: similarly presents with prodromal headache and bilateral exudative retinal detachments but typically occurs in pigmented individuals and does not have symmetric subretinal yellow deposits
  • Ocular lymphoma: typically presents with larger, more irregular and asymmetric subretinal deposits and is accompanied by vitreous cells
  • Best vitelliform dystrophy: fluorescein angiography typically shows blocked fluorescence from vitelliform lesion. There is often a positive family history.
  • Acute multifocal placoid pigment epitheliopathy: lesions also at the level of the retinal pigment epithelium, but usually more confluent. May be associated with headache, neck pain.
