Case 161 Diagnosis

Central serous chorioretinopathy

Differential Diagnosis (includes diseases that produce localized serous retinal detachments)

  • Optic nerve head pit: neither pinpoint leakage nor filling of the macular detachment on fluorescein angiogram
  • Choroidal hemangioma
  • Age-related macular degeneration: no late hyperfluorescence on fluorescein angiogram consistent with leaking choroidal neovascularization
  • Malignant hypertension, toxemia of pregnancy: no supporting history
  • Idiopathic choroidal neovascularization: no choroidal neovascularization, subretinal hemorrhages will not remit spontaneously
  • Presumed Ocular Histoplasmosis Syndrome: absent "punched-out" chorioretinal lesions, peripapillary atrophy, arcuate striae in mid-periphery
  • Harada’s disease: vitritis and systemic symptoms absent
  • Posterior scleritis: no vitritis or pain with ocular movement
