Kellogg Eye Center in Ann Arbor
We attempt to pre-register all patients scheduled at the Kellogg Eye Center location through a telephone interview three days before your scheduled appointment. If you have been pre-registered, you may proceed directly to the specific clinic where you will have your appointment. Your paperwork will be awaiting you there.

If you have not been pre-registered for your appointment, please check in at the registration desk before going to the clinic. The registration desk is located immediately inside the main doors of the two-story brick building, in the red waiting room.

Please arrive 15 minutes before your scheduled appointment time to complete the pre-appointment forms. You will receive a blue registration card. It is helpful if you bring this card with you to all future appointments with the U-M Health System.

When your registration is complete, you will be guided to the appropriate clinic, where we ask that you check in upon your arrival.
