What to bring with you

  • Your eyeglasses and/or contact lenses
  • A list of all current medications (including the name of each medicine and the prescribed dosage)
  • X-rays, CT scans, and any additional test results or copies of medical records you may have that relate to your condition.
  • Your blue hospital registration card.
    Returning patients of the U-M Health System should have one; new patients will receive a card during the registration process.
  • Your insurance card or the name and address of your insurance carrier, your policy number, the name of the policy holder, and your social security number.
  • Someone to drive you home. Some of our patients are given eye drops to dilate the pupils that may temporarily cause blurred vision. If you have had these eye drops at another examination, and they did not cause any blurred vision, you may drive yourself.
